To ensure that your request within the scope of Articles 11 and 13 of the Personal Data Protection Law No. 6698 can be fulfilled, please complete the following application form without omitting any data.

If you are going to send the application form by mail; send a wet-signed copy along with a notarized photocopy of your ID to the address of Fenomio Internet Reklamcılık Hizmetleri A.Ş. (hereinafter referred to as “Fenomio”) by writing “Personal Data Protection Law Information Request” on the envelope or delivering the application form in person with a document proving the identity of the applicant, by writing “Personal Data Protection Law Information Request” on the envelope.
If you are going to send the application form signed with a secure electronic signature or mobile signature, or using an email address previously notified to Fenomio and registered in the Fenomio system, you can send the application form to info@fenomio.com by writing “Personal Data Protection Law Information Request” in the subject line of the email.

Conclusion of Personal Data Owner Requests:

Your request will be answered as soon as possible and at the latest within thirty (30) days following the date it reaches us, in accordance with the Personal Data Protection Law. Our responses and evaluations will be sent to you in writing or electronically, as indicated in this Application Form, in accordance with Article 13 of the Personal Data Protection Law.

Your requests will be concluded free of charge by Fenomio, but if the response process incurs an additional cost, a fee may be charged as determined by the relevant legislation.

To ensure that your request is answered on time, you must complete all the information in this form without omitting any data. Otherwise, you will be asked to complete the missing information before you can be answered, which may delay the response time of your request.

Persons Submitting Requests on Behalf of the Personal Data Owner:

In requests made by third parties on behalf of the data owner, a notarized copy of the special power of attorney must be sent along with this Application Form. In requests made on behalf of children under guardianship/custody, a copy of the documents proving the guardianship/custody relationship must be sent along with this Application Form.

Declaration of the Personal Data Owner (Data Subject) / Representative:

I declare that I am legally authorized to make this request as the “Data Owner” or “Legal Representative of the Data Owner”, that the information and documents provided in the request are current and correct, and that I have not provided any illegal, incorrect, or misleading information.

I accept that, in case of making an unauthorized request, I may cause the illegal or unfair disclosure of information about personal data and that I will be responsible for any damage that may arise for Fenomio and the concerned persons.
