If submitting the application form by mail: Send the wet-signed form along with a notarized photocopy of your TC ID to Fenomio İnternet Reklamcılık Hizmetleri Tic. A.Ş.. (hereinafter referred to as "Fenomio") address, writing "Personal Data Protection Law Information Request" on the envelope, or if delivering the application envelope in person, come with an identification document and submit the application form in an envelope (writing "Personal Data Protection Law Information Request" on the envelope),

Resolution of Personal Data Owner's Requests: According to its nature, your request will be answered as soon as possible and at the latest within thirty (30) days following the date it reaches us in accordance with KVKK. Our responses and evaluations will be transmitted to you in writing or via electronic medium according to your selection specified in this Application Form, in accordance with Article 13 of KVKK.

Your requests will be concluded free of charge by Fenomio, and if the response process incurs additional costs, fees may be charged within the framework of relevant legislation.

To ensure your application is answered in a timely manner, you must complete all information in this form completely. Otherwise, missing information will need to be completed before we can respond to you. This may extend the response time for your application.

Persons Making Application on Behalf of the Data Subject (Related Person): For applications to be made by third parties on behalf of the data subject, a notarized special power of attorney must be submitted along with this Application Form, and for applications to be made on behalf of children under guardianship/custody, a copy of documents proving the guardianship/custody relationship must be submitted along with this Application Form.
